**VERBAL WARNING: issued for evidence of cooking beyond license agreement. Facility is a risk 3 which means no cooking can be done on site except for frozen pizzas. Please remove all utensils, warmers, roasters and various condiments that may indicate facility is cooking other foods besides pizzas. If facility is witnessed cooking other foods other than pizzas a $500 fine may be assessed and facility will be required to install a kitchen that is adequate to accommodate various cooking.
